Superbug hazard as medics 'stopped from cleaning ambulances'
    07/07/08 13:08
    Age: 8 days

    Published Date: 07 July 2008

    By GEMMA FRASER

PATIENTS are being exposed to potentially lethal superbugs because ambulances aren't being cleaned properly, it was claimed today.

Paramedics' leaders and those working on the frontline – including a crew member from West Lothian- have hit out at bosses, claiming their requests to "deep clean" vehicles after patients have been sick or had diarrhoea have been turned down.

They say they are not allowed to spend time cleaning the vehicles in case they compromise response targets. From April 2009, paramedics will have to answer 75 per cent of life-threatening emergencies within eight minutes.

The Scottish Ambulance Service is already gearing up to meet the target but paramedics say unacceptable corners are being cut, encouraging the spread of germs, including MRSA and clostridium difficile. Jonathan Fox from the Association of Professional Ambulance Personnel said: "The fight against superbugs cannot stop at the hospital door.

"The ambulance service is part of the total patient care package and nobody should have to be transported or work in a dirty ambulance. The issue of ambulance cleanliness is simply not being taken seriously enough."

A paramedic from West Lothian said: "We just don't have time to clean the vehicle – we're struggling. It's getting worse with the pressure of work and calls have gone through the roof.

"In this day and age, with the amount of infection going about, an ambulance should be deep cleaned at least once a week but that isn't happening."

One ambulance technician added: "The emphasis on the eight-minute target is the main problem. Our concern is that we are being refused permission to even clean vehicles between jobs, even if it's not fit for purpose."

The public service union Unison praised the practice adopted by the London Ambulance Service.

It has been described as leading the way in dealing with the problem with a "make-ready" system.

On-site cleaners routinely clean the fleet, work nights and carry out deep-cleans. They also re-stock ambulance with fresh kit so crews do not have to spend their time cleaning and re-stocking.

Labour MSP for Dunfermline East, Helen Eadie, has raised the cleanliness issue of Scottish ambulances with Health Secretary Nicola Sturgeon following complaints made by crews in her area.

A spokesman for the Scottish Ambulance Service insisted that adequate infection-control policies were in place.

He said: "Standards of infection control are very important to us.

"We require that vehicles should have both a daily and weekly clean, and that the interior of the vehicle is wiped down after each patient."

    www.frank-hdr.de/cgi-bin/lshop.cgi?ac...

    MRSA Figures Show A Drop

The number of infections from the superbug, MRSA, in English hospitals has fallen by more than a third over the past year, according to official figures.

Figures from the Health Protection Agency for April to June showed a 14% drop in hospital-acquired MRSA infections in England compared with the previous quarter, and a 36% reduction on the corresponding quarter of 2007.

The HPA's quarterly report found that 836 cases of MRSA were reported in England from April to June this year, compared to 969 in the previous quarter and 1,306 in the same period of 2007.

The fall in MRSA infections follows a drive to "deep clean" all NHS hospitals.

The total fall since 2004 is 57%, with the latest figures showing a 32% decline in another hospital-acquired infection, C difficile, over the past year.

"The reduction of healthcare associated infections is a big challenge throughout the world," aid the HPA's Professor Peter Borriello.

"The falls we are seeing in cases of MRSA bloodstream infections demonstrate the huge efforts being made by NHS staff to tackle these infections.

"The next challenge for the NHS will be to ensure that the downward trend continues and that we move to a position of zero tolerance.

Anna Walker, chief executive of the Healthcare Commission hailed the figures as "a big success in the war against MRSA".

However, she warned that there was still a variation in performance between different trusts, and that some still do not have the necessary systems in place to fight outbreaks.

"Some are falling far short of targets to reduce rates of MRSA," she said.

The Prime Minister has written to all NHS staff in a letter congratulating them on their achievement of exceeding their target of halving the cases of the potentially fatal hospital-acquired infection since 2004.

    MRSA cases down at last
    Friday, September 19, 2008, 10:00

HOSPITALS were yesterday celebrating after all but one of the Westcountry's healthcare trusts looked on track to beat their target for reducing cases of the superbug MRSA.
Figures released by the Health Protection Agency reveal that most of the hospitals in Devon and Cornwall have exceeded expectations for cutting cases of the blood stream infection.
The figures reflect a national drop in cases of 57 per cent compared to when the base line was set in 2003/4.
It prompted Prime Minister Gordon Brown to send out a letter of praise to all staff to thank them for the "tremendous achievement".
But the Conservatives pointed out that there were still more than 800 MRSA cases across the country over the last quarter – a situation they branded as a "disgrace".

But yesterday, health minister and Exeter MP Ben Bradshaw was upbeat about the achievements of staff during a visit to the Royal Devon and Exeter Hospital (RD&E), which has already exceeded its target of a 60 per cent reduction to hit 65 per cent.
He described MRSA as "the major concern" that patients had expressed about the NHS. He said: "These targets have really focused the minds of both hospitals and health care staff," he said. "When they were set many people said they could not be achieved, and it's fantastic that they have been."
But he recognised that more work still needed to be done – particularly at the Royal Cornwall Hospital (RCH), and said "difficult questions" needed to be asked. He said: "When Derriford, North Devon, Exeter and Torbay have all achieved such success, there's no reason why it can't be experienced everywhere."
Judy Potter, director of infection prevention and control at the RD&E, has visited the RCH to share her expertise.
Staff in Exeter are now carrying out a deep cleaning programme where wards are systematically decontaminated using a special dry mist.
The hospital is now aiming to eliminate all avoidable cases of infection by the year 2010, although Ms Potter said there will always be incidents when patients carry infection unknowingly. She said all hospitals should ultimately aspire to the same high goals. "If it's achievable for one, it's achievable for all," she said.
The Royal Cornwall Hospital Trust has been set a target of 23 cases this year – but has already reported 31 cases. Staff are responding by extending MRSA screening to all patients who come in for operations and more emergency admissions.
Julia Dutchman-Bailey, interim director of nursing at the Royal Cornwall Hospitals Trust, said fighting infection remained a "top priority".
She said the trust had worked to change antibiotic prescribing practices among hospital doctors and GPs, which is a key contributor in the rise of MRSA, and had adopted a "zero tolerance" approach to infection.
She said: "While any single infection is one too many, the numbers remain small in the context of more than 30,000 patients who will have been treated as day cases and inpatients in our hospitals over the same period."
In Torbay, the hospital is on track to have reduced MRSA numbers by 65 per cent in four years. Cases potentially acquired in hospital rather than brought in with the patient will have dropped by 80 per cent over that time scale.
Derriford is also on course to meet its target, but said there was work still to be done, with 18 cases so far this year. Dr Peter Jenks, director of infection control and prevention, said the hospital saw more cases than many others because of its size and the complicated cases it deals with. He said he was "pleased" with the result so far, and said the hospital was "well on the way" to achieving its own goal of all planned admissions being screened.
